Please bring your insurance card and photo ID to your appointment.

Insurance

We accepts Medicare, Medicaid and most major insurance plans. Please contact your insurance company to verify benefits prior to your visit and check to see if a referral is needed.  The review of your benefits should also include determining if we are in network or out of networkIf we are out of network, you will be responsible for any additional charges not covered by your insurance.

Your insurance may require you to pay a co-payment at the time of your appointment. (This is typically noted on the front or back of your insurance card.)  Copays must be paid at time of check in via check or credit card.  Note: We can not bill you for these copays.

Referrals

Many insurance companies are again requiring referrals from your primary care physician in order to see a specialist. Please familiarize yourself with your insurance company’s requirements. Should you have a plan that requires a referral, please request your primary care physician send via fax to 303-442-2931 this referral.  You too should have a copy and be prepared to provide the Referral number and the expiration date of the referral when calling for your appointment. Referrals must be in place prior to scheduling an appointment.
